Support for Adults Aged 65 and Over

Winsome Care Ltd provides specialist supported living services for older adults aged 65 and over across Liverpool and wider Northwest. We deliver compassionate, person-centred support that enables older adults to remain living in their own homes for as long as possible, avoiding unnecessary hospital attendance or admission to residential care.

Our approach is guided by NICE Guideline NG21 (Home Care: Delivering Personal Care and Practical Support to Older People Living in Their Own Homes) and the principles of the NHS Long Term Plan, which emphasise community-based support, prevention, and enabling people to maintain their independence and quality of life.

Our Approach to Supporting Older Adults

We understand that growing older does not mean losing independence, identity, or control. Our support is designed to complement the person's existing abilities and routines, not to replace them. We take the time to learn about each person's life history, preferences, routines, and the things that matter most to them, so that our support feels natural and respectful. For many older adults, maintaining social connections, staying active, and feeling valued are just as important as physical care. Our support addresses the whole person, not just their medical or physical needs.

What Our Support Includes

Falls Prevention

Risk assessments and prevention strategies informed by NICE Clinical Guideline CG161 (Falls in Older People: Assessing Risk and Prevention), including environmental assessments, exercise encouragement, medication reviews, and appropriate footwear and mobility aids

Nutrition and Hydration

Monitoring and support to prevent malnutrition and dehydration, with attention to dietary preferences, swallowing difficulties, and specialist dietary requirements

Medication Management

Support with medication administration, with a focus on polypharmacy awareness, potential interactions, side effects, and coordination with prescribers for regular medication reviews

Social Connection

Support to maintain social connections and prevent isolation and loneliness, including help to maintain contact with family and friends, access community groups, and participate in activities

Health Coordination

Liaison with district nurses, physiotherapists, occupational therapists, GPs, and hospital teams to ensure joined-up, coordinated healthcare

Personal Care

Sensitive personal care delivered with patience, kindness, and respect for the person's routines, preferences, and dignity

Domestic Support

Help with meal preparation, household tasks, shopping, and maintaining a clean, safe, and comfortable living environment

End-Of-Life-Care

Where appropriate, compassionate end-of-life support guided by the Gold Standards Framework, including advance care planning, symptom management coordination, and emotional support for the person and their family

Specialist Training for Older Adults

Staff supporting older adults receive specialist training in:

  • Falls prevention and safe mobility support
  • End-of-life care, advance care planning, and the Gold Standards Framework
  • Nutrition, hydration, and the prevention of malnutrition
  • Skin integrity, pressure area care, and wound awareness
  • Safeguarding of older adults, including recognising financial abuse and self-neglect
  • Loneliness and social isolation awareness
  • Communication with people experiencing hearing loss, visual impairment, or cognitive changes

Every older person we support has a lifetime of experience, knowledge, and stories. Our staff are trained to deliver support with patience, kindness, and deep respect for the person's life history, achievements, and identity. We never rush, never patronise, and never make assumptions about what a person can or cannot do based on their age. We support people to remain active, engaged, and valued members of their communities for as long as possible.
